Job Description

Position Overview: Beacon High School is seeking a Clinical Coordinator to launch and lead our Building Resilience for Youth in Transition (bryt) program, in partnership with The Brookline Center for Community Mental Health. The Clinical Coordinator will help to shape a new, innovative support system for students returning to school after experiencing significant mental health challenges. The Clinical Coordinator will support students’ emotional recovery, academic re-entry, and family engagement—working in close collaboration with school staff and community partners. This position is ideal for a skilled clinician who is passionate about adolescent mental health and systems coordination.

 

This is a full-time, tenure-track position in the tenure area of School Social Worker. The Clinical Coordinator will serve as a member of Beacon High School’s Student Services team, which includes a full-time School Psychologist and two additional full-time School Social Workers.

 

Qualifications:

  • Licensed Clinical Social Worker (LCSW) (required).
  • Valid New York State School Social Worker certification (required).
  • Formal training and experience working with adolescents and families struggling with complex challenges, including mental health challenges, causing extended absences from school.
  • Excellent case management skills.
  • Demonstrated skill and experience providing setting clinical goals and providing direct clinical supports to adolescents in the school setting, including collaborating with collateral/community-based mental health providers.
  • Demonstrated skill and experience working with parents/guardians from an assets/strengths-based perspective.

Responsibilities:

  1. Clinical Leadership and Student Support
     
  • Works with the Principal, other school leaders, and external support organizations to develop and continuously improve an intervention dedicated to successful transition back to full school participation on the part of students who have experienced significant mental health challenges and have missed extensive amounts of school time.
  • Provide individual and group support to students transitioning back to school after hospitalization or extended mental health-related absences.
  • Develop and oversee individualized re-entry plans for each student, incorporating clinical goals, academic accommodations, and social-emotional supports.
  • Serve as the primary clinical point of contact for students enrolled in the bryt program, ensuring continuity of care during reintegration.
  • Conduct risk assessments, crisis intervention, and safety planning when needed.

2. Care Coordination and Systems Navigation

  • Coordinate communication and planning among the school team, family, outside providers, and community-based mental health services to support each student’s holistic needs.
  • Attend IEP/504 meetings as appropriate and collaborate with special education and guidance staff to ensure services are aligned.
  • Maintain consistent follow-up with outpatient therapists, psychiatrists, and service agencies to ensure integrated care.

3. Family Engagement

  • Establish strong relationships with students’ families and caregivers through regular communication, outreach, and in-person meetings.
  • Educate and support families about mental health challenges, school resources, and strategies to foster resilience at home.
  • Facilitate connections between families and community supports, including family counseling, advocacy resources, and wraparound services.

4. School Collaboration and Integration

  • Work closely with the Academic Coordinator, teachers, administrators, school psychologist, school social workers, and guidance counselors to build a cohesive reintegration team.
  • Serve as a mental health resource and consultant to school staff, providing training and guidance on supporting students with emotional and behavioral needs.

5. Program Management and Development

  • Lead the daily operation of the bryt room, establishing routines, maintaining a therapeutic and structured environment, and managing the flow of students.
  • Identify trends and challenges across student cases to inform program development and improvement.
  • Participate in ongoing professional development through the Brookline Center’s bryt Network, and integrate best practices into local implementation.

6. Data Collection and Reporting

  • Maintain thorough documentation of services provided, progress notes, and care coordination efforts using the program’s secure data system.
  • Track student attendance, clinical goals, academic progress, and program outcomes for internal evaluation and external reporting.

7. Other Responsibilities

  • Provide additional direct and indirect School Social Work services to students as needed, including through counseling, crisis assessment and evaluation, and participation in IEP/504 meetings.
